Microsoft: Windows 11 might fail to start after installing KB5058405

Microsoft has confirmed that some Windows 11 systems might fail to start after installing the KB5058405 cumulative update released during this month's Patch Tuesday.

On affected devices, users see 0xc0000098 recovery errors in ACPI.sys, warning that the device has to be repaired because the operating system couldn't be loaded.

ACPI.sys is the Windows Advanced Configuration and Power Interface (ACPI) driver, a kernel-mode driver critical for power management and device configuration on Windows with an ACPI BIOS.

"We are investigating reports of the May 13, 2025 Windows security update (KB5058405) failing to install on some Windows 11, version 22H2 and 23H2 devices," the company said in a Wednesday update to the Windows release health dashboard.

"Your PC/Device needs to be repaired. The operating system couldn't be loaded because a required file is missing or contains errors," the error warns.

This known issue impacts Windows 11 22H2/23H2 systems in enterprise environments and mainly affects Azure Virtual Machines, Azure Virtual Desktop, and on-premises virtual machines hosted on Citrix or Hyper-V.

​Microsoft added that home users of Windows Home or Pro editions are unlikely to face these problems because the impacted virtual machines are mostly used in IT environments.

Redmond is investigating this issue and said its engineers will provide more information when it's available.

Earlier this month, the company fixed another known issue blocking Windows 11 24H2 feature updates via Windows Server Update Services (WSUS) after installing the April 2025 security updates.

Previously, it shipped emergency out-of-band updates to mitigate a Windows 10 bug that forced affected PCs into BitLocker recovery after installing the May 2025 security updates.

In April, Microsoft also addressed what it described as a "latent code issue" causing some devices to be upgraded to Windows 11 automatically despite Intune policies that should block Windows 11 upgrades.
